If you completed online enrollment for medical, dental, or vision insurance via UKG, you should receive a United Healthcare ID card at your home address within a few weeks.

If you do not receive your ID card, or if you have any questions, please contact United Healthcare directly via the myuhc.com website or by calling (800) 651-5465.

Please note it takes a few weeks from the time you complete your online enrollment until you show up as active in United Healthcare's system.
